Carelon Behavioral Health Coverage for Drug Rehab

Plans administered by Carelon cover detox and residential treatment. Carelon itself is not your insurer. It manages substance use benefits on behalf of other companies' plans, and it is not the Carelon company that reviews physical therapy. New Spirit Recovery treats members whose behavioral health runs through Carelon at its residential facility in Encino, Los Angeles, using their out-of-network benefit.

Two companies, one name

What does Carelon do, and which Carelon handles addiction treatment?

Carelon Behavioral Health reviews and authorizes mental health and substance use treatment. Carelon Medical Benefits Management, formerly AIM, reviews physical, occupational and speech therapy. They share a parent company and nothing else that matters to a rehab claim. Calling the wrong one costs days.

The two Carelon companies a member is likely to encounter.
Company What it reviews Governs a rehab claim
Carelon Behavioral HealthThis one Mental health and substance use treatment, from withdrawal management through outpatient care. Yes. Precertification, medical necessity, continued-stay review.
Carelon Medical Benefits Management Outpatient physical, occupational and speech therapy. Formerly AIM Specialty Health. No. A different benefit entirely, despite the shared name.

Carelon is an administrator, not an insurer.

Nobody buys a Carelon policy. An employer, a union welfare fund or a state programme buys health coverage, and then contracts Carelon to run the behavioral health part of it. So two people who both say they “have Carelon” can hold completely different benefits, because the plan behind Carelon is what sets the deductible, the coinsurance and whether an out-of-network benefit exists at all.

Carelon Behavioral Health was named Beacon Health Options until March 2023. A Carelon Behavioral Health drug rehab authorization can therefore appear under either name, since plan documents printed before then carry the Beacon name for the same benefit and the same review process.

New Spirit Recovery establishes which plan sits behind a member’s Carelon coverage before quoting anything, because the administrator’s name reveals nothing about the deductible or the out-of-network percentage underneath it.

Levels of care

What levels of addiction treatment does Carelon authorize?

Carelon authorizes withdrawal management and residential treatment where the clinical record establishes medical necessity, and steps the authorization down as the picture improves. New Spirit Recovery delivers those two levels on site in Los Angeles and refers the outpatient levels to providers near the member's home.

How Carelon treats each level of care, and how New Spirit Recovery delivers it.
Level of care How Carelon treats it At New Spirit Recovery
Withdrawal managementOn site Precertified and reviewed against withdrawal risk and medical stability. Delivered on site under 24-hour nursing. Typically 7 to 10 days, extending to 21 where the clinical picture requires it.
Residential treatmentOn site Precertified, then renewed through continued-stay review rather than granted as a block. Delivered on site in Los Angeles, typically as part of a 35-day programme.
Partial hospitalization A separate authorization, usually measured in programme hours per week. Referred to an external provider.
Intensive outpatient Authorized by session count rather than by day. Referred to an external provider.
Medication-assisted treatment Reviewed alongside the level of care it supports. Provided during treatment under medical direction, then coordinated at discharge.

Carelon's role and the Beacon renaming are stated by Carelon Behavioral Health. Facility figures come from New Spirit Recovery programme records. Administrator practice and facility practice are stated separately and never combined.

Four steps, in order

How does Carelon authorize residential addiction treatment?

Authorization runs on documented medical necessity, reviewed before admission and again throughout the stay. No plan approves a programme length up front, and a request that reaches Carelon after admission has already started is the most avoidable denial there is.

Step 1Identify the line

Reach Carelon, not the medical administrator

Substance use sits under behavioral health, so the number on the back of the card for mental health or behavioral health reaches Carelon. The general medical line administers a different benefit and cannot authorize a residential admission.

Step 2Precertification

The request goes in before admission

Carelon opens a precertification and asks the facility for the clinical picture: withdrawal risk, medical and psychiatric stability, relapse history, and what a less intensive setting would fail to address. An admission that starts before this is opened gives the plan grounds to deny the stay.

New Spirit Recovery opens the precertification before any travel is booked, which keeps the request from becoming the thing that delays an admission.

Step 3Medical necessity

A reviewer applies published criteria

Carelon measures the request against level-of-care criteria rather than against a diagnosis. The same substance and the same history can produce different decisions depending on what the record documents.

Step 4Continued stay

Authorization is renewed, never granted once

An approved admission is authorized for a period. Each review either extends it or steps the member down. Nobody receives a fixed programme length in advance, which is why any published day count should be treated as marketing rather than benefit.

New Spirit Recovery’s clinical team files every review and reports the outcome to the member the same day it arrives.

If you or someone else is in immediate danger, call or text 988 for the Suicide and Crisis Lifeline, available 24 hours a day in English and Spanish. SAMHSA's free treatment referral helpline is 1-800-662-4357. For a medical emergency, call 911.

The question behind the search

Does New Spirit Recovery need a Carelon contract to admit a member?

No. A plan with an out-of-network behavioral health benefit pays toward a facility holding no Carelon contract, at the out-of-network rate rather than a negotiated one. What decides it is your plan document, not the facility's contract status.

Most pages answering this question belong to facilities that are in network, so the answer they give is “choose us”. That is useful only if their bed, their programme and their location suit you. A member whose preferred facility is not on the list needs the other answer.

New Spirit Recovery holds no network contract with Carelon or with any other insurer. Members use the out-of-network benefit their plan carries. The admissions team reads that benefit from the policy on the first call and states what it pays before anyone commits or travels.

Two numbers decide the outcome: the percentage the plan pays out of network for behavioral health, and the out-of-pocket maximum that caps the year. Both come from the plan behind Carelon, not from Carelon itself. That out-of-network percentage is what makes a rehab center in Los Angeles reachable for a member whose local network holds no residential bed they want.

What to ask when you call Carelon

  • Does my plan carry an out-of-network behavioral health benefit?
  • What is my out-of-network deductible, and how much have I met?
  • What coinsurance applies to residential substance use treatment?
  • What is my out-of-pocket maximum for the plan year?
  • Has precertification been opened, and what does it still need?
  • Which level-of-care criteria apply to this request?

Ask for behavioral health, not general medical, or you reach an administrator that cannot authorize a residential stay. New Spirit Recovery's admissions line is (855) 932-2725. More on how coverage works and how admission works.

Questions Carelon members ask

Carelon and rehab, answered

What does Carelon do?
Carelon Behavioral Health manages mental health and substance use benefits on behalf of other companies' health plans. It is not the insurer. Your employer or state programme buys the coverage, and Carelon decides whether a treatment request meets medical necessity.
Is Carelon the same as Beacon Health Options?
Yes. Beacon Health Options was renamed Carelon Behavioral Health in March 2023, and it is part of Elevance Health. Members who still hold older plan documents will see the Beacon name on them. The benefit and the review process are the same.
Is Carelon Behavioral Health the same as Carelon Medical Benefits Management?
No. They are separate companies under the same parent. Carelon Medical Benefits Management, formerly AIM, reviews physical, occupational and speech therapy. Only Carelon Behavioral Health handles addiction treatment, so that is the line to call.
Is substance abuse considered behavioral health?
Yes. Health plans group substance use disorder with mental health under behavioral health, which is why a rehab request goes to Carelon rather than to the medical or hospital administrator. Asking the general medical line usually produces the wrong answer.
Does Carelon cover drug and alcohol rehab?
Carelon covers medically necessary substance use treatment, including withdrawal management and residential care, on plans that carry the benefit. What varies is the plan behind Carelon, because the employer or programme sets the deductible, coinsurance and network terms.
Do treatment centers need to be in network with Carelon?
No. A plan with an out-of-network behavioral health benefit pays toward a facility that holds no Carelon contract, at the out-of-network rate. New Spirit Recovery is out of network on every plan and reads that benefit from your policy before admission.
How do I verify my Carelon benefits for rehab?
Call the behavioral health number on the back of your insurance card and ask for your out-of-network benefit for residential substance use treatment. New Spirit Recovery runs the same check from the provider side during your first call.
How long will Carelon authorize residential treatment?
Carelon authorizes residential care in increments and extends it only while the clinical record supports continued stay. There is no fixed number of days set in advance. Each continued-stay review either extends the authorization or steps care down.
Why would Carelon deny a rehab request?
Most denials turn on documentation rather than exclusion: medical necessity not evidenced at the level requested, precertification opened late, or a reviewer judging a less intensive setting sufficient. A denial is appealable with the records the reviewer named.
How do I find rehab centers that accept Carelon in Los Angeles?
A Carelon rehab Los Angeles search returns directory listings, and Carelon rehab centers there hold contracts with the plan rather than with Carelon. Carelon drug rehab and Carelon alcohol rehab requests route through the behavioral line, never the medical one.
Does New Spirit Recovery work with Carelon-administered plans?
Yes, on an out-of-network basis. New Spirit Recovery holds no Carelon contract, so members use the out-of-network behavioral health benefit their own plan carries. The admissions team confirms which plan sits behind the Carelon card before quoting anything.
