UnitedHealthcare Coverage for Drug and Alcohol Rehab in Los Angeles

UnitedHealthcare pays for medically necessary withdrawal management and residential addiction treatment, once Optum has authorized it. Two things trip people up: the reviewer works at Optum, not UnitedHealthcare, and the word rehab names a second benefit entirely. New Spirit Recovery treats Optum-reviewed members at its residential facility in Encino, Los Angeles, using their out-of-network benefit.

One word, two benefits

Which UnitedHealthcare benefit are you asking about, and who reviews it?

On a UnitedHealthcare plan the word rehab names two unrelated benefits, reviewed by different people under different rules. Addiction treatment runs through Optum Behavioral Health. Physical rehabilitation runs through the medical benefit, and it is the one search results tend to return.

What each UnitedHealthcare benefit covers, and which entity decides it.
Benefit What it covers Who reviews it
Substance use and behavioral healthThis one Withdrawal management, residential treatment, partial hospitalization, intensive outpatient and therapy. Optum Behavioral Health, applying The ASAM Criteria to substance use decisions.
Physical rehabilitation Physical, occupational and speech therapy after a serious injury, surgery or illness. The medical benefit, under rules written for recovery from injury.
Medicare Advantage inpatient rehabilitation A semi-private room, meals, nursing and therapy in a rehabilitation facility, measured in days. Medicare rules rather than commercial plan rules. Not the addiction benefit.

Google’s own results steer this question to the wrong benefit.

UnitedHealthcare publishes a page about inpatient rehabilitation that covers physical therapy, occupational therapy, speech-language pathology, a semi-private room, meals and nursing. That page ranks first for several UnitedHealthcare rehab coverage searches, and it is a truthful page about the wrong benefit.

Ask does United Healthcare cover rehab and Google’s own AI Overview splits the answer in two, asking whether you mean substance use or physical rehabilitation. Most related searches alongside it concern Medicare day counts after a hospital stay.

A member who reads a day limit written for skilled nursing, and concludes their plan caps addiction treatment at that number, has read something accurate about a benefit they are not using. New Spirit Recovery opens every UnitedHealthcare verification with Optum’s behavioral line so the answer comes from the benefit in question.

The physical rehabilitation and Medicare inpatient rehabilitation benefits are described by UnitedHealthcare at uhc.com.

Levels of care

Which levels does Optum certify, and which does New Spirit Recovery run?

Optum states that certification is available for adult levels of care 3.1, 3.5 and 3.7, naming them in its published criteria. Two of those three are the levels New Spirit Recovery delivers on site in Encino, which is why the numbers on an authorization request matter more than the label a facility uses.

Which levels Optum certifies for adults, and which of them New Spirit Recovery runs in Encino.
Level of care How Optum treats it At New Spirit Recovery
Medically monitored withdrawal, ASAM 3.7On site Named as a certifiable adult level, assessed on withdrawal risk and medical stability. Delivered in Encino with nursing cover around the clock and physician oversight.
Residential treatment, ASAM 3.5On site Named as a certifiable adult level, assessed on the full set of ASAM dimensions. On site, continuing from withdrawal management without a transfer or a second intake.
Clinically managed low-intensity residential, ASAM 3.1 The third certifiable adult level, and a common step-down offer when 3.5 is denied. Referred, and named in the discharge plan before the member leaves.
Partial hospitalization A step down from residential, authorized separately from the levels above. Arranged with a step-down provider close to where the member lives.
Intensive outpatient Reviewed as a session allowance rather than as an admission. Handed over before discharge, with the receiving programme named.
Mental health without substance use Reviewed against LOCUS for adults 18 and over rather than against ASAM. Treated alongside addiction as dual diagnosis care, not as a standalone admission.

Four steps, in order

How does Optum authorize residential treatment on a UnitedHealthcare plan?

Optum applies The ASAM Criteria, Fourth Edition to adult substance use decisions, a standard it began implementing in November 2023, with one stated exception: state, federal or contractual requirements can mandate a different one.

Step 1The reviewer

Reach Optum's behavioral line, not the number on the card

UnitedHealthcare’s medical benefit cannot authorize a residential substance use admission. Optum Behavioral Health can, and UnitedHealthcare publishes a Substance Use Helpline on 1-855-780-5955 for members who want to start there.

New Spirit Recovery opens every UnitedHealthcare request with Optum and confirms which entity holds the behavioral review before submitting anything, because a member routed to the medical line loses days to a team that cannot decide the case.

Step 2The edition

The edition of ASAM in force decides how the request is read

Optum began implementing The ASAM Criteria, Fourth Edition for adults in November 2023, applying it to commercial health plans first. Other insurers still review against the Third Edition, so two members with the same diagnosis and different cards are measured against different editions of the same standard.

That is not a technicality. The Fourth Edition reorganised how levels of care are described, and a request written to the older structure answers questions the reviewer is no longer asking.

Step 3Your plan's state

Which state regulates your policy can override the rulebook

Optum applies ASAM except where state, federal or contractual requirements mandate otherwise, and California is one of the states with its own criteria. California Health and Safety Code section 1374.721 requires a health care service plan to apply the most recent criteria developed by the nonprofit professional association for the relevant specialty, which rules out proprietary internal guidelines.

The protection follows the policy rather than the treatment. New Spirit Recovery sits in California and admits members from every state, so it establishes which state regulates the plan before relying on any of it.

Step 4Continued stay

Authorization renews on the dimensions that opened it

Optum grants a window rather than a whole stay, and revisits it while treatment continues, on the same ASAM dimensions that opened the case. Nothing arrives as a block, which is why no published day count describes what a member receives.

New Spirit Recovery files each review, states which party is deciding it, and reports the outcome the same day.

If you or someone else is in immediate danger, call or text 988 for the Suicide and Crisis Lifeline, available 24 hours a day in English and Spanish. SAMHSA's free treatment referral helpline is 1-800-662-4357. For a medical emergency, call 911.

A question people actually ask

Why are providers leaving UnitedHealthcare's network, and what does it mean for rehab?

A provider leaving a network removes the contract, not the benefit. Network participation is a commercial agreement, and providers end it when reimbursement or administrative requirements stop working for them. The member's coverage moves to the out-of-network side of the plan rather than disappearing.

Residential addiction treatment is where the gap shows most. A plan’s behavioral network in a given city may hold few residential beds, or none at the level a clinician is asking for, and a waiting list is not a clinical answer for someone in withdrawal.

Out-of-network status is what lets New Spirit Recovery admit a member Optum has reviewed, from any state, without waiting for a network bed. Cost then attaches to the level Optum authorizes rather than to the admission, so a stay certified at 3.7 and one certified at 3.1 draw different reimbursement.

New Spirit Recovery holds no contract with UnitedHealthcare or Optum, and quotes no reimbursement percentage. Members admit on the out-of-network benefit written into their own plan, and it is that benefit that opens a drug and alcohol rehab in Los Angeles to them.

What to ask Optum

  • Am I speaking to Optum Behavioral Health rather than medical benefits?
  • Does my plan carry an out-of-network behavioral health benefit?
  • Which edition of the ASAM Criteria applies to my plan?
  • Which state regulates my policy, or is it self-funded?
  • What is my out-of-network deductible, and how much is met?
  • Has prior authorization been opened, and what does it still need?

Questions UnitedHealthcare members ask

UnitedHealthcare and rehab, answered

Does UnitedHealthcare cover drug and alcohol rehab?
Yes. Medically necessary withdrawal management and residential addiction treatment are covered benefits, with prior authorization opened before admission. Optum Behavioral Health reviews the request rather than UnitedHealthcare's medical line. New Spirit Recovery establishes the out-of-network side of that benefit from the policy itself.
Are UnitedHealthcare and Optum the same?
Not the same. Both belong to UnitedHealth Group, and Optum Behavioral Health manages the behavioral health benefit for most UnitedHealthcare commercial plans. The card carries the UnitedHealthcare name, while the reviewer deciding a residential admission works at Optum. Calling the number on the card frequently reaches the medical benefit instead.
What is Optum behavioral health?
Optum Behavioral Health is the UnitedHealth Group business that administers mental health and substance use benefits. It publishes the clinical criteria its reviewers apply, operates the provider network behavioral claims run through, and decides prior authorization and continued stay for residential treatment.
Which clinical criteria does Optum use for addiction treatment?
Optum states that from November 2023 it applies The ASAM Criteria, Fourth Edition to adult substance use decisions, unless state, federal or contractual requirements mandate otherwise. For mental health it applies LOCUS to adults aged 18 and over. Optum certifies adult levels of care 3.1, 3.5 and 3.7 by name.
How do I find a rehab that accepts UnitedHealthcare?
Coverage attaches to the level of care rather than to a building, so rehab facilities that accept United Healthcare include out-of-network ones, and a rehab that takes United Healthcare may hold no Optum contract at all. Optum certifies levels 3.1, 3.5 and 3.7.
Why are doctors no longer accepting UnitedHealthcare insurance?
Network participation is a contract, and providers leave when reimbursement or administrative requirements stop working for them. A provider leaving a network removes no part of a member's benefit. It moves that care to the out-of-network side of the plan, which is what New Spirit Recovery admits members under.
How many days of residential treatment will Optum authorize?
Optum authorizes a period and reviews it as the stay runs, against the same ASAM dimensions used to approve the admission. No fixed day count applies, and any published figure describes someone else's plan. Continued stay depends on what the clinical record supports at each review.
Does California law change my UnitedHealthcare rehab coverage?
Only if California regulates your policy. Health and Safety Code section 1374.721 requires a California plan to apply the most recent criteria from the nonprofit professional association for the relevant specialty, barring proprietary substitutes. A plan another state regulates, or a self-funded employer plan, sits outside it.
How do I find detox centers that accept United Healthcare?
United Healthcare drug rehab, United Healthcare alcohol rehab and detox all run through Optum rather than the medical line. Withdrawal management is covered where the record supports it, with prior authorization opened before admission.
Why do searches about UnitedHealthcare rehab coverage return physical therapy?
Because rehab names two separate benefits on a UnitedHealthcare plan. UnitedHealthcare's own page about inpatient rehabilitation covers physical, occupational and speech therapy after injury, surgery or illness, and it ranks first for several coverage searches. Day counts written for that benefit describe nothing about addiction treatment.
Is New Spirit Recovery in network with UnitedHealthcare or Optum?
No. New Spirit Recovery holds no contract with UnitedHealthcare, Optum or any other insurer. Members admit at the out-of-network rate, on the behavioral health benefit their own plan carries. The admissions team establishes what that benefit pays before anyone commits to travelling.
Is a rehab that accepts United Healthcare in Los Angeles open to out-of-state members?
Yes, where the plan carries an out-of-network behavioral health benefit, because that benefit follows the member rather than the state. New Spirit Recovery is in Encino, Los Angeles and coordinates travel, including flights.
