Does UMR Insurance Cover Drug Rehab?

Usually, but your employer decides what the plan covers. UMR administers plans that employers fund themselves, so the company you work for writes the benefit and UMR applies it. New Spirit Recovery treats members on UMR-administered plans at its residential facility in Encino, Los Angeles, using whatever out-of-network benefit the employer bought.

Same three letters

Is UMR the same as UnitedHealthcare?

No. UMR is a UnitedHealthcare company, but it administers plans rather than insuring them. A third organisation with the same initials, a physical rehabilitation hospital, is not related to either. Searching for UMR rehab returns all three, which is why the word insurance matters in the search.

Three organisations a member searching for UMR rehab will encounter.
Name What it is Relevant to a rehab claim
UMRThis one United Medical Resources, a UnitedHealthcare company. A third-party administrator for employers that fund their own health plans. Yes. It processes the claim and applies the employer’s rules.
UnitedHealthcare An insurer. It sells coverage and carries the financial risk itself, with behavioral health through Optum. Only if your plan is UnitedHealthcare rather than UMR-administered.
United Medical Rehabilitation Hospitals United Medical Rehabilitation Hospitals, a physical rehabilitation provider in Louisiana. Unrelated to insurance or addiction treatment. No. It appears in results purely because of the initials.

Why two people with a UMR card can have opposite coverage.

UMR administers thousands of separate employer plans. Each employer funds its own claims and writes its own rules, including the deductible, the coinsurance and whether an out-of-network behavioral health benefit exists at all. The card is printed the same way for everyone. What sits behind it is not.

So a published figure for what “UMR covers” describes one employer’s plan and tells you nothing reliable about yours. The document that holds your terms is the Summary Plan Description, issued by your employer, not by UMR. Human resources or the benefits portal has the current version.

New Spirit Recovery reads the specific plan behind a member’s UMR card rather than working from anything published about UMR generally, which is the only way to give a member on a self-funded plan an accurate number.

Levels of care

What levels of addiction treatment do UMR plans cover?

Self-funded plans treat detoxification and residential treatment as behavioral health benefits requiring precertification, then step the authorization down. New Spirit Recovery delivers those two levels on site in Los Angeles and refers the outpatient levels to providers near the member's home.

How a UMR-administered plan typically treats each level, and how New Spirit Recovery delivers it.
Level of care How a self-funded plan treats it At New Spirit Recovery
DetoxificationOn site Precertified. Reviewed against withdrawal risk, with the employer’s criteria applied. Delivered on site under 24-hour nursing. Typically 7 to 10 days, extending to 21 where the clinical picture requires it.
Residential treatmentOn site Precertified, then renewed by continued-stay review. Day counts are set per plan, not by UMR. Delivered on site in Los Angeles, typically as part of a 35-day programme.
Partial hospitalization A separate authorization. Some employers exclude it entirely. Referred to an external provider.
Intensive outpatient Authorized by session count. Commonly the most generously covered level. Referred to an external provider.
Sober living Housing rather than a medical benefit. Self-funded plans rarely fund it. Coordinated as part of discharge planning.

How self-funded administration works is described generally and applies to UMR-administered plans as a class. Your own figures come from your employer's Summary Plan Description. Facility details come from New Spirit Recovery programme records, and the two are stated separately throughout.

Who decides, in order

How does a UMR plan authorize residential treatment?

Four parties touch the decision, and UMR is only one of them. The employer sets the rules, a reviewer applies them, the authorization runs in increments, and the employer's own fund pays the claim. Knowing which party is deciding is what makes an appeal work.

Step 1Your employer

The benefit is written before UMR sees it

A self-funded employer decides what its plan covers, including whether an out-of-network behavioral health benefit exists at all. UMR is handed those rules and applies them. No call to UMR can add a benefit the employer did not buy, which is why the Summary Plan Description matters more than anything UMR publishes.

Step 2Precertification

UMR opens the review against those rules

The facility submits the clinical picture and UMR measures it against the plan’s medical necessity criteria. Some employers route behavioral health to a separate vendor, so the number on the card may reach a reviewer that is not UMR at all.

New Spirit Recovery confirms which entity actually holds the behavioral health review before submitting, rather than discovering it after a denial.

Step 3The determination

Approval is for a period, not a programme

An authorized admission covers a set number of days, reviewed as it runs. Because the employer funds the claim, a self-funded plan can be stricter or more generous than a comparable insured plan with the same card on the front.

Step 4Payment

The employer's money pays the claim

UMR processes and pays from the employer’s fund rather than from an insurer’s reserves. Practically, that means appeals can involve the plan administrator at your employer as well as UMR, and it is worth asking who has final say before an appeal is filed.

New Spirit Recovery files the appeal and names which party is deciding it.

Find your real terms

How do I verify UMR coverage for rehab?

Get your Summary Plan Description from your employer first, then call the behavioral health number on your card. The document tells you what was bought. The phone call tells you what remains of it this plan year.

Ask specifically whether behavioral health is administered by UMR or routed to a separate vendor. Self-funded employers frequently carve behavioral health out to a different company, and a member who spends a week talking to the wrong reviewer has lost a week for no reason.

New Spirit Recovery establishes which entity holds the behavioral health review before it submits anything, then reads the out-of-network benefit from the policy and states what it pays. It holds no network contract with UMR or with any other administrator.

Where an employer bought an out-of-network benefit, that percentage is what makes a recovery center in Los Angeles reachable for someone whose local network has no residential bed they want.

What to ask, and who to ask

  • Human resources: may I have the current Summary Plan Description?
  • UMR: is behavioral health administered by you, or carved out to another vendor?
  • Does the plan carry an out-of-network behavioral health benefit?
  • What is the out-of-network deductible, and how much is met?
  • What coinsurance applies to residential substance use treatment?
  • Who has final say on an appeal, UMR or the plan administrator?

GEHA and other association plans are often UMR-administered, so the same questions apply. New Spirit Recovery's admissions line is (855) 932-2725. More on how coverage works and how admission works.

If the answer is no

What does New Spirit Recovery do if UMR denies the treatment?

A denial on a self-funded plan is appealable, and the appeal may be decided by your employer's plan administrator rather than by UMR. That is the difference that matters here: on an insured plan the insurer has final say, and on a self-funded plan the company you work for often does.

So the first question after a denial is not what the reason was, it is who owns the decision. UMR can tell you which criteria it applied. Only the plan administrator can tell you whether the employer will exercise discretion. Asking UMR to overturn something the employer wrote is a wasted round.

What you pay when coverage does apply is the deductible you have not met plus coinsurance, capped by the out-of-pocket maximum. Every one of those three numbers was chosen by your employer, which is why they vary so widely between two people holding the same card.

New Spirit Recovery files the appeal, names which party is deciding it, and quotes cash pay in full alongside it so both figures sit side by side rather than arriving weeks apart.

What an appeal needs

  • The written denial, naming the criteria applied and by whom
  • The clinical records the reviewer said were missing
  • The Summary Plan Description section the denial relies on
  • Confirmation of who decides the appeal, UMR or the plan administrator
  • The filing deadline, which the plan document sets rather than UMR

Cash pay is quoted in full before admission. See how coverage works for when paying cash costs less than using a benefit.

Questions UMR members ask

UMR and rehab, answered

Is UnitedHealthcare and UMR the same thing?
No. UMR is a UnitedHealthcare company, but it administers self-funded employer plans rather than insuring them. UnitedHealthcare sells insurance and carries the risk. UMR processes claims against money the employer has set aside, under rules the employer wrote.
How does UMR insurance coverage for rehab work?
Your employer funds the plan and writes the benefit, and UMR applies it. UMR rehab coverage therefore differs between two members holding the same card, because the employer rather than UMR decided what to buy.
Will UnitedHealthcare pay for rehab?
If your plan is UMR-administered, UnitedHealthcare is not the payer. Your employer's own fund pays the claim and UMR processes it. That distinction matters at appeal, because the final say may rest with your employer's plan administrator rather than an insurer.
How long does UnitedHealthcare pay for rehab?
Neither UnitedHealthcare nor UMR sets a fixed length. Authorization runs in increments and continues while the clinical record supports it. On a self-funded plan any maximum is written in your employer's Summary Plan Description, not published by the administrator.
Does UMR insurance cover alcohol rehab?
Alcohol rehab covered by UMR follows the employer's plan document rather than a UMR rule. UMR addiction treatment coverage commonly includes withdrawal management and residential care, but the out-of-network share is the employer's choice.
Why do two people with UMR get different answers?
Because they work for different employers. UMR administers thousands of separate plans, each written by the company funding it. The card looks identical and the benefit underneath can differ completely, which is why a published UMR coverage figure means very little.
Where do I find what my UMR plan actually covers?
In your Summary Plan Description, the document your employer issues rather than anything UMR publishes. It states the out-of-network percentage, the deductible and the precertification rules. Human resources or your benefits portal holds the current version.
Is UMR the same as United Medical Rehab Hospital?
No, and the initials cause real confusion. United Medical Rehabilitation Hospitals is a physical rehabilitation provider in Louisiana with no connection to insurance. UMR, meaning United Medical Resources, administers health benefits. Searching UMR rehab returns both.
Does UMR cover rehab out of state?
That depends on whether your employer purchased an out-of-network benefit. Where it exists, the benefit follows you rather than the state, so a plan written in Ohio or Texas can pay toward residential treatment in California.
Does UMR cover rehab if I relapse?
A relapse is not itself an exclusion, and a readmission is precertified the same way a first admission is. Some plans apply a waiting period after a discharge against medical advice, written in the Summary Plan Description rather than set by UMR.
Is New Spirit Recovery in network with UMR?
No. New Spirit Recovery holds no network contract with UMR or any other administrator. Members use whatever out-of-network behavioral health benefit their employer's plan carries, read from the policy on the first call.
Will my UMR plan pay for a facility in Los Angeles?
If your employer bought an out-of-network behavioral health benefit, it pays toward a Los Angeles facility on the same terms as a local one. New Spirit Recovery is in Encino and verifies that benefit from your policy on the first call.
