Surest Coverage for Drug and Alcohol Rehab in Los Angeles

Surest plans cover medically necessary detox and residential treatment for substance use. How Surest prices that care is what makes it unusual: no deductible, no coinsurance, and one fixed copay the member reads in the app before admission. That copay is a network price. New Spirit Recovery treats Surest members out of network at its residential facility in Encino, Los Angeles.

One number, not three

Why does Surest quote one copay for an entire rehab stay?

Surest removes the deductible and the coinsurance and prices care as a fixed copay instead, and for residential treatment Surest states that the copay runs from admission to discharge rather than accruing per day. Surest publishes that rule in its own member guide. Almost nothing written about Surest and rehab mentions it.

What a member owes for residential treatment under each pricing model.
The number On a deductible plan On a Surest plan
Deductible Owed in full before the plan pays toward anything. None. Surest states the plan starts working on day one.
Coinsurance A percentage of every authorized day of the stay. None. Surest administers the plan without coinsurance.
Residential priceThe difference Deductible, then a percentage of each day, capped by the out-of-pocket maximum. One copay covering the stay from admission to discharge.
Outpatient price The same three numbers applied to each visit. A copay per day for partial hospitalization and intensive outpatient.
When the member learns it After the claim is processed, on an explanation of benefits. Before care is booked, by searching the Surest app.

The copay in the app is a network price.

Surest builds those copays around the UnitedHealthcare and Optum Behavioral Health networks, and it states the rule for providers plainly: a facility reaches Surest’s network by holding a contract with one of those two networks rather than by signing anything with Surest itself.

New Spirit Recovery holds no UnitedHealthcare contract and no Optum contract, so a Surest member admits out of network. Surest names three consequences of that choice: costs may rise, balance billing may occur, and the member is expected to manage prior authorizations. Only the third is something a facility can take off the member.

Surest insurance coverage for rehab raises one more distinction the search results confuse. Surest is employer-sponsored coverage, which makes a Surest card commercial insurance rather than a public programme, and New Spirit Recovery admits adults on commercial plans.

The copay structure, the residential rule and the network rule are published by Surest at surest.com.

Levels of care

What does a Surest plan pay at each level of addiction treatment?

Surest assigns a copay to each level of care and shows it before booking, pricing residential treatment once for the stay and outpatient programmes by the day. The two levels New Spirit Recovery delivers on site in Encino are the two Surest prices as a single figure.

How Surest prices each level of care, and how New Spirit Recovery delivers it.
Level of care How Surest prices it At New Spirit Recovery
Withdrawal managementOn site Priced as an inpatient admission, with the copay covering the duration of the stay. On site in Encino under 24-hour nursing, with physician oversight throughout.
Residential treatmentOn site One copay from admission to discharge, listed in the app under residential treatment facility care. On site, continuing from detox without a transfer or a second intake.
Partial hospitalization A copay applied per day rather than once for the programme. Referred to a provider near the member’s home.
Intensive outpatient A copay applied per day, on the same basis as partial hospitalization. Referred, and named in the discharge plan before the member leaves.
Substance use disorder clinic Priced for medication treatment of opioid use disorder, including methadone. Referred where a member continues medication after discharge.
Sober living Housing rather than treatment, so no copay reaches it. Arranged in discharge planning and quoted separately.

Four steps, in order

How does a Surest plan authorize residential treatment, and who files it?

Surest requires prior authorization for admission to some substance use programmes, and network status decides who opens it. Surest states that in-network providers contact Surest to begin the process, and that a member choosing to go out of network should expect to manage it themselves.

Step 1The app price

Read the residential copay in the Surest app first

Surest directs members to search residential treatment facility care in the app to view the benefit and the copay attached to it. That figure arrives before anyone is admitted, which is unusual and worth using.

New Spirit Recovery asks Surest members to pull that number early, because it establishes what the plan considers an in-network residential stay to be worth. It is the figure everything else gets compared against.

Step 2The contract

Establish whether the facility holds a UnitedHealthcare or Optum contract

Surest states the test for providers directly: a provider contracted with the UnitedHealthcare or Optum Behavioral Health networks is in network for Surest members. Surest itself does not sign the contract that creates that status.

New Spirit Recovery holds neither contract, so the app copay is not the price that applies. What applies comes from the out-of-network behavioral health benefit written into the plan, and reading it takes a member ID and a phone call.

Step 3Who files it

Prior authorization moves to whoever will file it

Surest requires prior authorization for admission to some substance use programmes, and says in-network providers contact Surest to initiate it. Its member guidance is equally clear that going out of network can leave the member managing those authorizations.

New Spirit Recovery files the request instead, submits the clinical record that supports the level of care, and tracks every continued-stay review. Precertification for Surest behavioral health runs through Optum on 1-877-237-0006.

Step 4The balance

Balance billing is a risk Surest names, so name it back

Surest lists balance billing among the consequences of choosing an out-of-network provider. A member is entitled to ask what the plan pays and what remains, and to get both figures before travelling rather than after discharge.

New Spirit Recovery states the expected out-of-network figure and the cash figure together, on the first call. It quotes no reimbursement percentage for any Surest plan, because its records document none.

If you or someone else is in immediate danger, call or text 988 for the Suicide and Crisis Lifeline, available 24 hours a day in English and Spanish. SAMHSA's free treatment referral helpline is 1-800-662-4357. For a medical emergency, call 911.

The number that actually applies

What does the Surest app copay mean at New Spirit Recovery?

The copay shown in the Surest app is the price of in-network care, so it is not the figure that applies at a facility holding no UnitedHealthcare or Optum contract. What applies instead sits in the plan's out-of-network behavioral health benefit, and New Spirit Recovery reads it before anyone travels.

Surest is direct about the trade. Its member guidance states that choosing to go out of network may cause costs to increase, incur balance billing, and require the member to manage prior authorizations. That sentence names three separate things, and only one of them is about money.

The authorization burden is the one a facility can lift, and New Spirit Recovery files it for out-of-network Surest members. No reimbursement percentage appears here for any Surest plan, because its records document none. With no deductible to time, the cash and benefit figures stay stable whenever a member calls.

New Spirit Recovery holds no contract with Surest, UnitedHealthcare or Optum. Members use the out-of-network behavioral health benefit their own plan carries, and that benefit is what puts a drug and alcohol rehab in Los Angeles within reach of someone whose network holds no residential bed they want.

Questions Surest members ask

Surest and rehab, answered

Does Surest cover rehab, and how is it priced?
Yes. A Surest health insurance rehab claim covers medically necessary withdrawal management and residential care, with prior authorization opened before admission. Surest drug rehab coverage and Surest detox coverage are priced as copays rather than as a deductible plus coinsurance.
Is Surest insurance the same as UnitedHealthcare?
Not the same, though they are related. Surest is a UnitedHealthcare company that administers a plan with no deductible and no coinsurance, and it draws on the UnitedHealthcare and Optum Behavioral Health networks. A UnitedHealthcare plan carries a deductible and a coinsurance percentage. The card tells you which one you hold.
What is the difference between a PPO and a Surest plan?
A PPO prices care as a deductible followed by a coinsurance percentage, and it usually carries a separate out-of-network tier. Surest prices care as a fixed copay with no deductible and no coinsurance. That distinction matters at an out-of-network facility, because a copay is a network price and a coinsurance percentage is not.
Does Surest insurance cover therapy?
Yes. Surest states that behavioral health and therapy services carry fixed copays, and members view the copay for a given provider before booking. Outpatient programmes are priced per day, while residential care is priced once for the whole stay.
Why is Surest so cheap?
The design removes the deductible and the coinsurance rather than reducing what is covered, and it sets each price in advance so members see it before booking. For a residential admission the practical effect is one number instead of three.
How can I check my Surest eligibility?
Surest members present the Surest member ID card before receiving treatment, and Member Services answers on 866-683-6440, Monday to Friday, 6am to 9pm Central. New Spirit Recovery verifies the benefit itself with a member ID, usually inside ten minutes, and reports what the policy says rather than what the app estimates.
How do I find a rehab that accepts Surest?
Search the app for residential treatment facility care to see the network copay, then ask whether the plan carries an out-of-network benefit. Surest residential treatment coverage reaches beyond the providers the app lists.
Who files the prior authorization if the rehab is out of network?
Surest states that in-network providers contact Surest to start that process, and that choosing to go out of network may require the member to manage prior authorizations. New Spirit Recovery files the request for out-of-network Surest members instead, tracks each continued-stay review and reports the outcome the same day.
Was Surest previously called Bind?
Yes. The plan launched as Bind in 2016, and UnitedHealth Group states the change to the Surest name began to take effect on 1 September 2022. Older documents, directories and provider listings still carry the Bind name, so a plan described as Bind and a plan described as Surest can be the same coverage.
Is New Spirit Recovery in network with Surest?
No. Surest treats a provider as in network when that provider holds a UnitedHealthcare or Optum Behavioral Health contract, and New Spirit Recovery holds neither. Members admit out of network and use whatever out-of-network behavioral health benefit their plan carries.
Can I use a Surest plan for rehab in Los Angeles if I live in another state?
Yes, where the plan carries an out-of-network behavioral health benefit, because that benefit follows the member rather than the state. Surest is employer-sponsored coverage, so it travels with employment. New Spirit Recovery is in Encino, Los Angeles and coordinates travel, including flights and airport transportation.
